Add form fields and styling to Slick Contact Form WP Plugin

I have a Wordpress template I'm working on for a client and I have downloaded the Slick Contact Forms plugin. It's a plugin that floats on the page and slides out when the contact tab is clicked. Right now, the plugin limits you to three text fields, but I need to add more, for a total of 6 text input fields, a captcha and a comments box. I also need a bit of styling and positioning of the button itself. I've attached some screenshots of the current button and contact form functionality. I've also attached the client's original form, which we need to create, but in the new style I've created.
