Survey Development Tool

This should be a fairly easy project for someone that knows exactly what they are doing. I am needing a dynamic survey development tool for my core eCommerce system. I need to have this done in CGI, ColdFusion, PHP or .NET. I prefer .NET and will accept PHP as a second option. I am running Windows servers but want this application to run on Windows or Linux systems. You must supply good documentation for installation of the application. If you preview <[login to view URL]> and take a look at their standard and professional versions of Survey Solutions, this will give you a good idea of what I am looking for. I want this application to be client and server admin. I want my hosting customers to be able to create surveys for their web sites. I also want the admin side to control all functions and accessibility of my clients. In other words, the entire system must be dynamic and I must have control over enabling or disabling client use of the survey or formbuilding function. You can track, enable or disable through IP or any type of secure authentication method. Please do not bid telling me that you can do this project. If you have done a forum building or survey type of tool for your previous clients, please do not bid. You do not have to supply design unless you want to include that in your bid. I already have a designer and a sample design that we follow on most of our other software apps. This is going to be a rebrandable application and will be sold as a standalone application and incorporated into my suite of products. Thanks!
