Daycare and Gymnastics site needed

I need a website designer/developer to help create a website for a Daycare center. Here are the needed feature: -Site will have a tab on the daycare other business ? gymnastics. We'll need to develop a 5 page site for the gymnastic since it will has it own domain and hosting outside the daycare. - We'll need you to create us a logo -Ability for parent to register for gymnastic and an online pre-enrollment form for the daycare ? no need for online payment at this time. -There will be a clickable button that will be used for calling the daycare. -An online pre-enrollment form. -Forms must be completed online and email to daycare or printed.
