Convert Excel Spreadsheet Calculator to Web App/Widget + Remake Sign Up Form

I have a loan calculator and repayment calculator in excel that I need to integrate into an application form on a Responsive HTML website. Need the application form recreated in PHP or other format that emails all form data + uploaded files to an email address upon submission. Must be functional on mobile devices (apple/android) + all major browers IE, Chrome, Opera, Firefox, Safari - so no flash conversions. Prefer either PHP script or Ajax form either direct inserted or iframe integration if need be. Job requires quick turnaround of 2 days and bonus paid for completion by Monday 9am. Offering $150 base rate for development and site integration + $250 bonus for completion prior to Monday 13th July 9am.
