Internal webapp to sync Jotform data to MySQL

Our company software uses MySQL as its database. Currently we manually enter the registration data from JotForms into our company software. I would like an application that takes the data from JotForms using its API/HTTPGET ([login to view URL]) and allow us to compare the data side by side with the patients information in our MySQL 5.5 database. Id like to be able to see what lines are identical and which ones have changed. We should be able to review the form data, edit and needed, and then click a button to update the patients data in our database so that we dont have to do it manually anymore. I am able to write the queries on the MySQL side so you dont need to learn all of the database tables. I will let you know exactly where the data links in the database. I would like this to be a webapp, and this will only be internal. Patients will never use this. This will have patient information so we need to make this HIPAA compliant. I would like this configurable on my end so we dont have to share database details. We will develop this using a test demo database with the same structure but with generic patient data. Over time our forms will change so this needs to be able to be developed in a way that lets us link more forms and to setup future database fields/links ourselves without having to always involve you.
