Small PHP + MYSQL project.

Hello, I need to create a small website in php with mysql database as follows: Main page: To show contents of database table with options to search by words inside the table columns and to display only those rows containing those words also option to sort by column field contained in row for example in table we will have a column Location, so we need to be able to select from a dropdown one of the lcoations and to display only records containing that location. Admin page where we can add data to the database, modify or delete. Database format: columns: Photo First Name Last Name Phone Email Location Custom1 Custom2 Custom3 Custom4 On the admin page when we add data we should have this: Besides name etc which is self explanatory we need the following: Location field needs to be check box with 5 locations to select from and it needs to be able to select multiple locations for each. For example we want person1 to show up in all 5 locations or only in 2. Custom1 field needs to be 5 drop down lists which will contain 100 options to chose from, and all 5 selected options need to be added to the database column for that row, it can also be just 1 or 2 selections done the other can be empty/nothing selected. Custom2 field needs to be 15 drop down lists which will contain 100 options to chose from, and all 5 selected options need to be added to the database column for that row, it can also be just 1 or more selections done the other can be empty/nothing selected. Custom3 field should be 5 drop down lists which will contain 20 options to chose from, and all 5 selected options need to be added to the database column for that row, it can also be just 1 or 2 selections done the other can be empty/nothing selected. Custom4 field should be text box to enter text and add it max 255 characters. Main page should have css for custom styling , we need to be able to add a logo, modify background and all colors around, it should have a friendly design, nothing fancy. Admin page can be without a nice design.
